Read the instructions at the top of the request form carefully each time because these instructions change periodically.

  1. Whose name and phone number goes into the form?
Put the name and phone number of an adult who will be riding. If the client is a minor, please put their name under “Name and age of each person” who will be riding. You can put as many phone numbers as you’d like in the form, including your own.

  2. What if my client only has Whatsapp?
We are able to communicate with clients using Whatsapp.

  3. Whose email should I put in the form?
Put the email of the person who needs to receive an immediate automated confirmation that the request was received. Often this is the email of the caseworker or provider. We will call the client directly to set up a pick up time.

  4. Do I put in when I want the client picked up or the client’s appointment time?
Put the exact time of the appointment in the form. We need this in order to call the client and determine an accurate pick up time.

  5. Do you arrange the ride with the client or with me?
The majority of the time, we arrange the ride with the client so we can confirm their contact info and pick up address. We will try to reach them by phone, text and voicemail. If we get no response, we will try to let you know in a timely manner.

  6. How does the client arrange for a ride home?
The client will call us when they are completely finished with their appointment and we will arrange for the ride back.

  7. What can I do if my client doesn’t have a car seat?
Whenever we set up rides with clients with children, we determine whether or not they have car seats they need by law.

  8. What is “Message to MANA” for?
This is where you can tell us anything else specific about the ride.

“Thank you for making sure our patients get to and from their appointments safely!”
